Please do not submit Social Security numbers, payment card numbers, or detailed diagnoses through website forms. Prefer calling (714) 790-4085 for sensitive conversations.
How website forms are delivered
Refill and transfer forms are emailed to the pharmacy using FormSubmit, a third-party form relay. That means the message passes through FormSubmit’s systems before it reaches our pharmacy inbox. This convenience path is not a HIPAA-certified patient portal. A secure clinic ordering portal is planned separately from this marketing site.
How we use the information
We use form submissions to respond to refill or transfer requests, clarify details, and coordinate pickup. We do not sell your contact information.
Analytics and cookies
This marketing site does not currently load Google Analytics tags. If analytics are added later, we will update this notice.
